Dental Networks

First Dental Health (FDH) is a network of dentists, selected by dentists, offered to a variety of customers including individuals/families, administrators, employers and insurance companies. Today FDH has our own networks in Arizona, California and Nevada. To learn more about our networks and to receive participation information please use the request information page.

Networks

• PPO Network
  A fee for service network, leased by over 40 different plan administrators, employers, and insurance companies. Participation in this network affords your office, exposure to over 2,500 different employers (350,000 lives).


• EPO (PPO PLUS in AZ,NV) Network
  A subset of the PPO network and is also fee for service based with a deeper discount than the PPO. Employer groups may chose this network for their employees as a stand-alone or as a point of service product.


• ACCESS Network
  Is the network, which is available to those individuals/families who do not have insurance. Patients in this program would not have claim forms, deductibles or waiting periods to meet! You simply treat the patient and collect directly from the patient the FDH Access contracted fees.
