Privacy policy

The privacy and protection of your information is important to First Dental Health® (“FDH”).  This privacy policy explains how we protect and use the information that we collect from you.  

What we collect.

 If you are a member of, or are applying to join, a FDH network, we collect the following information: 

  • Identity Information: We collect information used to identify you, such as your name, date of birth, social security number, and gender. 
  • Contact Information:  We collect information used to stay in contact with you, such as your physical address, email address, and telephone number. 
  • Professional Information:  We collect information about your professional history and dental practice, such as your education and training history, work history, DEA information, licensing information, insurance information, malpractice history, and information that you authorize us to collect from others. 

If you are interacting with us, such as through our website, email or phone, we collect the following information: 

  • Identity Information:  We collect information that you provide to us through forms, email or other communication, such as your name. 
  • Contact Information:  We collect information that you provide to us through forms, email or other communication, such as your physical address, email address, and telephone number. 
  • Website Use Information:  We collect information about how you interact with our website, such as what pages you access or visit, the domain you are accessing our website from, and information that you provide us in response to things such as surveys and site registrations. 

How we collect your information. 

The information we collect is either provided by you or is collected from third parties that you have authorized us to contact as part of the application process. 

How we use your information.  

If you are a member of, or are applying to join, a FDH network: 

  • Your Identity, Contact and Professional Information are used to evaluate your application/participation in the FDH networks. If you become a member of a FDH network, your Identity and Contact information are used to provide you the services and support set forth in the Provider Agreement. Your Professional Information will be used to evaluate your ongoing eligibility within the FDH networks as part of our re-credentialing process. We do not sell your personal information. 

If you are interacting with us, such as through our website, email or phone: 

  • Your Identity and Contact Information are used by us to provide you with the information, support or services that you requested. Your Website Use Information is used to improve the content and layout of our website, to provide you with customized content, to notify you about updates to our website, and to help ensure the security of our website. We do not sell your personal information. 

Who we share your information with. 

If you are a member of, or are applying to join, a FDH network: 

  • We share information with third parties that you have provided written authorization for when applying to join a FDH network.  
  • We share information with third parties as set forth in the Provider Agreement. For example, you agree that we can publish your name and address in our directory that is made available to potential and existing FDH members. 
  • We share information with our service providers that help us provide you with the services and support set forth in the Provider Agreement. 

If you are interacting with us, such as through our website, email or phone: 

  • We share information with our service providers and affiliated companies that help us provide you with the information, support or services that you requested. 

Website cookies. 

Our website uses cookies to store visitors’ preferences, record session information and record aggregated information on what pages users access or visit. You can manage cookies through your internet browser. 

How we protect your information. 

The security and confidentiality of your information, and the information of your patients, is extremely important to us. We use industry-standard encryption technologies when transferring and receiving personal information. And we have appropriate security measures in place to protect against the loss, misuse or alteration of personal information that we have collected from you. Our information security program is designed to comply with all applicable federal and state laws, and to comply with the requirements set forth in the Provider Agreement. 

Rights under California law. 

Depending on the nature of your relationship with us, and the type of information we collect about you, you may have certain rights under California state law. This could include the right to request that we disclose what personal information we have collected, used or disclosed about you. You may also have the right to request that we delete personal information that we have collected or maintain about you. To submit a request for yourself or on behalf of an individual, please email us at providerrelations@firstdentalhealth.com, call us at 1-858-689-0904, or use our online Contact Us form (use “Rights Request” in the Subject field). We will review your request and respond in a timely manner. If the request is valid, we will contact you to further verify your identity, using information that we have on file. You will not be discriminated against for seeking to exercise any rights provided to you under California state law.
